Book excerpt: Aortic diseases (mainly including aortic dissection and aneurysm) are common but life-threatening entities. With development of surgical and endovascular techniques, treatment outcomes have been improved gradually. However, a lot of problems still require to be resolved. Aortic diseases in the region of visceral arteries are difficult to treat, especially for aortic dissection, which is usually associated with the narrow true lumen. Conventional open repair is associated with high trauma and high morbidity and mortality. Endovascular intervention has been applied in the last decade. Fenestration technique, chimney technique, and branched stent-grafts have been used for aortic aneurysms. For aortic dissection, management in the region of the visceral arteries is more complicated, and more techniques have been described. Besides these three methods, exclusion of the re-entry by vascular occluder, road block strategy, and candy-plug technique are also reported. However, these endovascular techniques vary in characteristics, procedures, and outcomes. There is no consensus on the optimal treatment for these complicated entities.
